Here's what I came up with.
Card stock - So Saffron, Baha Breeze, Soft Suede (Stampin' Up)
Decorative Paper - All Patched Up (Karen Foster)
Stamp Set - On the Mend (Lawn Fawn)
Ink - Soft Suede (Stampin' Up)
Die - Label (Spellbinders)
Embossing Folder - Perfect Polka Dots (Stampin' Up)
Instructions
1. Create card from card stock - finished size is 4.25 x 5.5
2. Cut yellow panel and adhere to card
3. Cut decorative paper panel and adhere to card
4. Run Baha Breeze and So Saffron through die machine
5. Dry emboss larger label die with polka dot embossing folder
5. Stamp image and sentiment on yellow panel
6. Adhere die cuts together and adhere to card
